Chronic Disease Self-Management Program or PATH (Personal Action Toward Health)
PATH (Personal Action Toward Health) is a chronic disease self-management program that helps participants build the skills they need for the day-to-day management of a chronic disease. PATH is a six-week workshop and covers topics including healthy eating, relaxation techniques, problem solving and communication skills.

This low-cost, evidence-based exercise program helps older adults at all levels of fitness become more active, energized, and empowered to sustain independent lives. Water is a safe, ideal environment for relieving arthritis pain and stiffness. That's why the Arthritis Foundation offers its Aquatic Program, designed to safely keep your joints moving and improve your sense of well-being.
