Chronic Disease Self-Management Program or PATH (Personal Action Toward Health)
PATH (Personal Action Toward Health) is a chronic disease self-management program that helps participants build the skills they need for the day-to-day management of a chronic disease. PATH is a six-week workshop and covers topics including healthy eating, relaxation techniques, problem solving and communication skills.

Enhance Fitness
This low-cost, evidence-based exercise program helps older adults at all levels of fitness become more active, energized, and empowered to sustain independent lives. Based on solid research and tested at over 80 sites around the country, EnhanceFitness focuses on stretching, flexibility, balance, low impact aerobics, and strength training exercises – everything health professionals say that people need to maintain health and function as they grow older.

Aquatics Program
This warm-water exercise program, suitable for every fitness level, has been shown to reduce arthritis pain and improve overall health. Exercises include range of motion, muscle-strengthening, socialization activities and an optional, moderate-intensity aerobic component.

Led by trained leaders, the classes meet two to three times per week for 6-10 weeks. Each session provides a friendly and supportive environment that encourages social interaction. Although the classes are held in the water, swimming skills are not necessary.

The goal of this program is to provide you with the tools for being more active in your own home whether you're new to working out or already exercising regularly.
